- Secure transmission of patient-generated health data (PGHD).
- Clinical oversight and analysis by licensed providers.
- Alerts for out-of-range readings or concerning patterns.
- Educational and care coordination support.
The Services are intended to supplement — not replace — traditional in-person medical care.
3. User Responsibilities
- Follow all instructions for device setup and usage.
- Regularly use the device(s) as prescribed.
- Notify your provider of any device malfunctions, changes in condition, or other issues.
- Maintain the confidentiality of your login credentials.
- Refrain from using the Services in any unlawful or unauthorized manner.
Failure to adhere to these responsibilities may result in suspension or termination of your access.
4. Medical Disclaimer
Not for Emergencies: Hayes RPM is not designed for use during medical emergencies. Call 911 or go to the nearest emergency room.
No Physician-Patient Relationship: Use of the Hayes RPM website does not in itself establish a physician-patient relationship.
Provider Oversight Required: All care decisions are made by your licensed provider. Hayes RPM is a technological enabler, not a medical provider.
